Job summary

The role requires developing performance improvement plans, auditing records, and ensuring regulatory compliance, with travel to branches as needed to drive improvements and share knowledge among clinical staff.

Qualifications

• 3 years of certified home health experience required
• Value Based Purchasing, PDGM, CAHPS-preferred
• Licensure, Certification, Registration
• CT Licensure in field of specialty
• Commitment to regulatory requirements and quality improvement

Responsibilities

• Develop and implement performance improvement plans.
• Review clinical records for compliance and accuracy.
• Analyze quality data and generate reports for leadership.
• Coordinate with staff, regulatory bodies, and payors.
• Educate staff on best practices and quality improvement methodologies.
• Travel to branches as needed to support sites.
