Job summary

Hartford HealthCare at Home in Mansfield, CT is seeking a QAPI Specialist to advance clinical documentation and outcomes using a disciplined, data-driven approach. You will compile trends, report on quality metrics across patient care, safety, risk management, infection control, outcomes and patient satisfaction, and lead the development of Performance Improvement Plans.

You will review audits, ensure ICD-10 coding relevance, and implement interventions to improve quality scores.

Qualifications

• 3 years of certified home health experience required.
• Knowledge of Value Based Purchasing, PDGM, CAHPS-preferred.
• Licensure, Certification, Registration in CT required.
• Experience in data analysis and quality reporting.

Responsibilities

• Assist with process improvement plans and PI teams.
• Compile trends and report quality data across patient care and safety.
• Lead development of Performance Improvement Plans from audits.
• Review OASIS and ensure coding accuracy.
• Develop interventions to improve quality scores.
• Coordinate with agency to ensure compliance with regulations.
• Travel to branches as needed.
