Job summary

You will lead IDG meetings, mentor team members, and ensure compliance with Medicare/Medicaid and ACHC standards while improving performance and efficiency at your location.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care, including 2 years in hospice or home-based care.
• Ability to ensure regulatory compliance (Medicare, Medicaid, JCAHO, ACHC).
• Comfort with providing compassionate care to terminally ill patients and families.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling, including reassigned, missed, declined, or rescheduled visits.
• Triage incoming referrals and medical concerns promptly and professionally.
• Lead or facilitate IDG meetings, present patient updates, coordinate care plans, document minutes.
• Mentor and train associates, supporting development and compliance.
• Ensure all care delivery meets regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiency at the location.
