Job summary

Advance Hospice Care seeks a dedicated Patient Care Manager (RN) to lead clinical operations for our hospice team. You will oversee patient care delivery, coordinate visits, triage referrals, and ensure compliance with regulatory standards.

Reporting to the Executive Director, you will mentor staff, facilitate IDG meetings, and drive performance improvement while maintaining compassionate, patient-centered care.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care experience, including 2 years in a hospice or home-based care setting.
• Demonstrated knowledge of Medicare/Medicaid, JCAHO, ACHC regulatory standards.
• Ability to lead a diverse care team while adapting to varied patient needs.
• Strong interpersonal, organizational, and leadership skills.
• Competency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure delivery of high-quality hospice care.
• Manage visit scheduling and triage referrals based on clinical priority.
• Lead or facilitate Interdisciplinary Group (IDG) meetings and document minutes.
• Mentor, train, and support associates to improve performance and compliance.
• Ensure care delivery meets federal, state, and local regulatory standards and company policies.
• Support performance improvement, quality assessments, and operational efficiencies at the location.
