Gentiva is seeking a Registered Nurse (RN) Manager for its Savannah, Georgia hospice team. This leadership role oversees patient care, directs clinical operations, and coordinates care plans while ensuring regulatory compliance and exceptional service delivery.

The position reports to the Executive Director or Administrator. The Nurse Manager leads an interdisciplinary team, mentors staff, and drives performance improvement to maintain high standards of patient care and regulatory adherence

Formación

• RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care, incl. 2 years in hospice/home health.
• Knowledge of Medicare/Medicaid, JCAHO, ACHC regulations.

Responsabilidades

• Oversee patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize clinical needs.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings and coordinate care plans, documenting minutes.
• Mentor and support staff, ensuring compliance and performance.
• Ensure care delivery meets regulatory standards and company policies.
• Support quality improvement initiatives and operational efficiency.
