Job summary

SouthernCare, Inc. is seeking a Patient Care Manager to lead clinical operations within our hospice team. You will oversee patient care delivery, manage visits, triage referrals, and coordinate care plans.

The role requires an active RN license in the state, hospice experience, and strong leadership. You will mentor staff, ensure regulatory compliance, and drive quality improvement initiatives.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care experience, including 2 years in hospice or home-based care.
• Knowledge of hospice principles, Medicare/Medicaid, and regulatory compliance.
• Compassion for terminally ill patients and their families.
• Ability to lead a diverse care team with empathy and resilience.
• Strong interpersonal, organizational, and leadership skills.
• Competency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure the delivery of high-quality hospice care.
• Manage visit scheduling, including reassigned, missed, declined, or rescheduled visits based on clinical judgment.
• Triage incoming phone referrals and medical concerns in a timely, compassionate, and professional manner.
• Lead or facilitate Interdisciplinary Group (IDG) meetings, including presenting patient updates and coordinating care plans.
• Mentor, train, and provide clinical support to associates to develop performance and compliance.
• Ensure all care delivery meets federal, state, and local regulatory standards and company policies.
• Support quality improvement initiatives, performance improvement and operational efficiencies.
