Job summary

Heartland Hospice seeks a Patient Care Manager to oversee clinical operations, coordinate care delivery, and ensure compliant hospice services. This role leads the care team, manages visits, and upholds regulatory standards across Medicare/Medicaid guidelines.

Reporting to the Executive Director, you will mentor clinicians, optimize schedules, and drive quality improvement within the location.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care, including 2 in hospice/home health.
• Knowledge of Medicare/Medicaid, JCAHO, ACHC standards.
• Strong leadership, interdisciplinary team coordination, and documentation skills.

Responsibilities

• Oversee patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritization based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings, present updates, coordinate care plans, document minutes.
• Mentor and support staff to promote performance and compliance.
• Ensure care meets regulatory standards and company policies.
• Support performance improvement and operational efficiency at location.
