Heart’n Soul Hospice is seeking a Patient Care Manager - Registered Nurse to lead clinical operations and ensure high-quality, compassionate care for patients facing life-limiting illness. The role focuses on coordinating departments, supervising nursing staff, and maintaining compliance with state and federal regulations.

Qualified applicants hold a current RN license, have hospice RN experience, and demonstrate strong organization and communication skills.

Qualifications

• Current State Registered Nurse license
• Two years of hospice RN experience in clinical care or home health
• Strong organization, people and problem solving skills
• Ability to multi-task across projects in a multidisciplinary setting
• Excellent critical thinking and communication skills

Responsibilities

• Direct and coordinate clinical departments and sites, ensuring continuity, quality, and safety of services per regulatory standards.
• Supervise nursing staff and guide team to meet plan of care and reassess patient needs.
• Support education to enhance knowledge and train staff.
• Provide coaching, feedback and annual evaluations to team members.
• Timely submission of all documentation and adherence to policies.
