Job summary

Houston Hospice is seeking a Patient Care Manager (RN) to provide clinical and operational leadership for an interdisciplinary team delivering high-quality hospice care. You will oversee admissions, plans of care, and service delivery while maintaining regulatory standards and budgetary goals.

The role requires strong leadership, coaching, and communication skills, with a focus on patient- and family-centered care across a fast-paced environment.

Qualifications

• Current, unrestricted Texas RN license.
• BSN from an accredited nursing program; Bachelor’s degree preferred.
• Minimum of five years of management experience, preferably in hospice, palliative care, home health, or a related healthcare setting.
• Clinical experience in hospice, home health, or public health nursing.
• Strong leadership, coaching, problem-solving, and decision-making abilities.
• Excellent communication, organizational, and interpersonal skills.
• Knowledge of federal and Texas regulations governing hospice care.
• Ability to travel throughout the service area and participate in on-call coverage.
• Current CPR certification.
• Reliable transportation, valid driver’s license, and required automobile insurance.

Responsibilities

• Lead and develop interdisciplinary team (nurses, hospice aides, social workers, chaplains) and promote accountability and collaboration.
• Oversee patient admissions, ongoing assessments, plans of care, and timely delivery of services.
• Monitor documentation, care coordination, and clinical outcomes to ensure regulatory compliance and quality care.
• Lead interdisciplinary team (IDT) meetings and strengthen communication with staff, patients, families, and partners.
• Coordinate staffing, assignments, productivity, and resources while maintaining budgetary expectations.
• Identify opportunities to enhance workflows and improve patient and family experiences.
• Serve as a clinical resource and participate in the administrative on-call rotation.
