Job summary

Gentiva Hospice seeks an experienced Senior Hospice RN Patient Care Manager to lead care operations, supervise staff, and ensure regulatory compliance across our hospice program. The role reports to executive leadership and coordinates interdisciplinary care plans to maintain high-quality patient outcomes.

Requirements include an RN license in the state of employment, an accredited nursing degree, and a minimum of three years direct patient care with hospice knowledge.

Qualifications

• RN license in the state of employment.
• Graduate of an accredited nursing program (Assoc. degree accepted with experience).
• Minimum 3 years direct patient care experience.
• Knowledge of hospice principles and regulatory compliance.
• Strong communication, collaboration, and organizational skills.
• Proficiency with EMR and healthcare software.
• Valid driver’s license and auto insurance.
• CPR certification.

Responsibilities

• Lead hospice patient care operations to ensure quality and compliance.
• Supervise, teach, evaluate, and develop nursing staff.
• Manage referrals, DME pickups, Medicare eligibility, and patient benefits.
• Coordinate IDG meetings to review care plans and update documentation.
• Oversee scheduling and timely completion of visits.
• Respond to referrals and clinical inquiries professionally.
