Gentiva is seeking a dedicated Hospice Patient Care Manager to lead clinical excellence and team performance in delivering compliant hospice services. You will oversee patient care activities, coordinate visits, triage referrals, and guide interdisciplinary teams to ensure exceptional patient outcomes.

Ideal candidates hold an RN license, with 3+ years in direct patient care including hospice/home health, and a BSN preferred.

Qualifikationen

• RN license in the state of employment
• Graduate of an accredited nursing program
• Minimum of 3 years of direct patient care experience, including 2 years in hospice or home-based care
• Knowledge of hospice principles, regulatory compliance (Medicare/Medicaid/JCAHO/ACHC)
• Compassion for terminally ill patients and families
• Ability to lead a diverse care team with sensitivity to different backgrounds
• Strong organizational and leadership skills
• Clinical documentation proficiency

Aufgaben

• Oversee and direct patient care services to ensure high-quality hospice care
• Manage visit scheduling including reassigned, missed, declined, or rescheduled visits based on clinical judgment
• Triage incoming referrals and medical concerns promptly and compassionately
• Lead or facilitate Interdisciplinary Group meetings and coordinate care plans
• Mentor, train, and provide clinical support to associates
• Ensure care delivery meets regulatory standards and company policies
• Support performance improvement and operational efficiencies at the location
