Job summary

Gentiva is seeking a dedicated Patient Care Manager to lead our hospice team in Arlington Heights, IL. You will oversee patient care, coordinate visits, and mentor staff to uphold the highest standards of regulatory compliance and compassionate service.

In this leadership role, you will coordinate IDG meetings, triage referrals, and ensure documentation accuracy across systems, while advancing performance improvement initiatives and operational efficiency.

Qualifications

• Current RN license in the state of employment.
• Minimum of 3 years of direct patient care experience, including 2 years in hospice or home-based care.
• Knowledge of hospice regulatory standards (Medicare, Medicaid, JCAHO, ACHC).
• Strong leadership and mentoring skills.
• Ability to coordinate care and supervise a diverse team.
• Proficient in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize visits based on clinical judgment.
• Triage incoming referrals and medical concerns promptly.
• Lead Interdisciplinary Group meetings and document minutes.
• Mentor staff and support development and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support quality improvement initiatives and operational efficiency.
