Qualifications

• Excellent observation, verbal and written communication skills and problem solving.
• Experience in hospice, home health or community health preferred.
• Licensed driver with reliable transportation and CPR certification.
• Management experience not required; may supervise hospice aides
• Offer contingent on references and criminal background checks
• Excellent observation, verbal and written communication, problem solving and math skills
• Prolonged walking/standing; able to lift/transfer patients and equipment
• Licensed driver with insured vehicle in good condition
• CPR certification required (hands-on verification)

Responsibilities

• Completes comprehensive patient/family assessments to determine hospice needs.
• Provides nursing care using the nursing process.
• Develops and revises care plans with patient/family input.
• Administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing needs and self-care.
• Acts as case manager coordinating care for assigned caseload.
• Maintains accurate clinical notes and communicates with physicians.
• Participates in on-call duties and ensures equipment and services are available.
• Supervises ancillary personnel and pursues ongoing professional development.
