Job summary

Affinity Health Management seeks an experienced RN to plan, organize, and direct hospice care in patients' homes and communities, leveraging nursing expertise to support families. The role emphasizes education, resource coordination, and high-quality end-of-life care.

The RN will perform assessments, develop and revise care plans, administer treatments, and educate patients and families while collaborating with the interdisciplinary team to meet patient needs.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing; CPR certification required.
• Preferred: BSN or higher with public health/home care experience.
• Minimum clinical experience within last three years; hospice exposure preferred.
• Ability to supervise aides and coordinate patient care.

Responsibilities

• Complete initial and ongoing patient/family assessments for hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient/family involvement.
• Administer prescribed medications and treatments.
• Educate patients and families on health management and pain control.
• Coordinate care as case manager with the interdisciplinary team.
