Job summary

Affinity Health Management is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings. The nurse will use community resources to meet patient and family needs and will work within an interdisciplinary team.

Responsibilities include comprehensive patient assessments, care planning, administering medications, caregiver education, and coordinating care as case manager. Candidates must have an active RN license and CPR certification.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing to practice.
• 1+ year recent nursing experience; public health/home care/hospice preferred.
• CPR certification required.
• Excellent observation, communication, problem solving skills.

Responsibilities

• Perform comprehensive patient and family assessments to determine hospice needs.
• Develop and revise care plans with patient and family involvement.
• Administer medications and nursing treatments as prescribed.
• Coordinate care as case manager with the clinical team.
• Educate patients and families on self-care and palliative care.
• Maintain accurate clinical notes and documentation.
• Participate in on-call duties and team meetings.
