Job summary

Silverado Hospice in the San Diego area is hiring an RN Case Manager to coordinate care, develop individualized care plans, and deliver hands-on hospice and palliative care for patients and families. This role focuses on quality end-of-life care and support throughout the patient journey.

You will lead with clinical expertise, collaborate with the Interdisciplinary Group, and supervise LVNs and Home Health Aides in the field.

Qualifications

• Active RN license in the state of California.
• Minimum 1 year RN experience; hospice experience preferred.
• Ability to lift up to 50 pounds.
• Must clear criminal background check, physical, and drug screening.
• Valid driver’s license with reliable transportation.

Responsibilities

• Develop detailed Plans of Care for assigned patients, ensuring services are identified and met.
• Coordinate care with Interdisciplinary Group, patient, family, and attending physician.
• Provide skilled hospice and palliative care for the remainder of the patient’s life.
• Conduct comprehensive nursing evaluations and assessments.
• Provide field supervision to LVNs and Home Health Aides assigned to your patients.
• Perform field case management and other duties assigned.
