Apploi is seeking a registered nurse for hospice care, directing services in home and community settings. You will plan, organize, and direct patient care, focusing on family-centered education and support within the home.

Primary duties include comprehensive assessments, care planning, medication administration, and coordination with the interdisciplinary team to meet evolving patient needs.

Competenze

• Graduate of an accredited school of nursing with an active RN license in good standing.
• Minimum one year of nursing experience within the last three years, or a BSN with two years’ experience (public health/home care/hospice preferred).
• Excellent observation, verbal and written communication skills; strong problem solving and basic math abilities.

Mansioni

• Completes comprehensive patient and family assessments to determine hospice needs.
• Delivers professional nursing care using the nursing process and develops/updates the plan of care.
• Initiates and revises care plans as patient needs change; coordinates with interdisciplinary team.
• Administers medications and treatments as prescribed; counsels patient and family on care needs.
• Completes clinical notes and communicates with physicians; ensures safe environment and continuity of care.
