Job summary

Amedisys Home Health in Norfolk, Nebraska seeks a Nurse Practitioner to deliver high-quality home-based care as a PRN clinician. You will assess patients, manage symptoms, and coordinate with the interdisciplinary team to support healing at home.

This role offers flexible scheduling, competitive pay, and benefits through the company's plans; telehealth access; PTO; and retirement options. Candidates must be licensed NPs with Medicare billing privileges and CPR certification.

Qualifications

Current and unencumbered Nurse Practitioner license in the state of assignment.
Minimum of one year NP experience or one year hospice RN experience in last three years.
Authority to practice and prescribe as NP where services are provided.
Medicare billing privileges as NP.
Knowledge of Medicare reimbursement and coding across home/office/nursing settings.
Current CPR certification.

Responsibilities

Communicate significant findings and changes in condition to the clinical manager/physician and interdisciplinary team.
Conduct face-to-face encounters and assess disease-related changes, functional decline, adherence, and QoL.
Order and review labs, X-rays, EKGs and other diagnostics; document treatment plan and review results with collaborating physician.
Make referrals to other disciplines as indicated by patient needs.
Actively participate in interdisciplinary team conferences.
