Job summary

Amedisys Home Health is seeking a PRN Nurse Practitioner to provide patient-centered care in home settings. You will perform face-to-face visits, assess conditions, coordinate care with the clinical team, and support palliative treatment plans.

This role emphasizes flexibility and ongoing professional growth in a trusted home health environment. Requirements include an active NP license, ability to prescribe, Medicare billing privileges, CPR certification, and at least one year of NP or hospice

Qualifications

• Current and unencumbered NP license in the state of assignment.
• Minimum of one year experience as a Nurse Practitioner or hospice RN in the last three years.
• Must have authority to practice and prescribe as NP in the state of services.
• Must have Medicare billing privileges as NP.
• Must meet state NP collaboratives and provide physician agreements.
• Must have insurance as required by state law for NP practice or be eligible via employer coverage.
• Knowledge of Medicare reimbursement and coding for home-based services.

Responsibilities

• Communicate significant findings to the clinical manager and interdisciplinary team.
• Conduct face-to-face encounters assessing disease progression, function, and quality of life.
• Order/review labs, imaging and diagnostics; document treatment plans with collaborating physician.
• Make referrals to other disciplines as needed.
• Participate actively in interdisciplinary team conferences.
