Amedisys is hiring a Nurse Practitioner for hospice care, PRN as needed, delivering compassionate home-based services to patients and families. The role emphasizes collaboration with the clinical team, assessing symptoms, and coordinating with physicians to ensure comfort and quality of life.

Qualified candidates will hold an active NP license, have Medicare billing privileges, and meet state collaboration requirements.

Qualifications

• Active Nurse Practitioner license in the state of assignment.
• 1 year of NP or hospice RN experience within last 3 years.
• Ability to practice and prescribe as NP per state regulations.
• Medicare billing privileges for NP required.
• CPR certification current.

Responsibilities

• Communicate significant findings to the clinical manager and interdisciplinary team.
• Complete face-to-face encounters and assess disease changes and QoL.
• Order/review labs, X-rays, and tests; document treatment plans.
• Make referrals to other disciplines as needed.
• Participate in inter-disciplinary team conferences.
