Amedisys Home Health is seeking a Nurse Practitioner to deliver home‑based care, perform face‑to‑face assessments, and coordinate with a multidisciplinary team to optimize patient comfort and function at home.

The role requires a valid NP license in the target state, Medicare billing privileges, and state‑specific collaborative agreements. CPR certification is required; prior hospice experience is a plus.

Qualifications

• Current and unencumbered NP license in the assigned state.
• Minimum of one year NP experience or hospice RN experience in the last three years.
• Authority to practice and to prescribe as a NP in the applicable state.
• Must have Medicare billing privileges as a NP.
• Must meet state collaborative requirements and provide physician collaboration agreements.
• Maintain insurance as required by state law for NP practice or be covered by employer.
• Knowledge of Medicare reimbursement and coding for various service levels.
• Current CPR certification.

Responsabilités

• Communicate significant findings, problems, changes in condition or environment to the clinical manager/attending physician and the interdisciplinary team.
• Complete face-to-face encounters and assess disease-related changes, function decline, adherence to regimen, and quality of life.
• Order and review labs, x-rays, EKGs and other diagnostics; document and review treatment plan with collaborating physician.
• Make referrals to other disciplines as indicated by patient needs.
• Actively participate in interdisciplinary team conferences.
