Qualifications

Current and unencumbered Nurse Practitioner license in the state of employment.
Medicare billing privileges as a Nurse Practitioner.
Authority to prescribe as a Nurse Practitioner.
CPR certification.
Experience in hospice or end-of-life care.

Responsibilities

Communicate significant findings and changes to the clinical team.
Perform face-to-face encounters and assess patient condition and pain/symptom management.
Order and review labs, imaging, and diagnostics for palliation and treatment.
Make referrals to other disciplines as needed.
Participate in interdisciplinary team conferences.
