Job summary

Amedisys is seeking a Nurse Practitioner to provide hospice care with a strong focus on symptom management and patient comfort in home settings. You will collaborate with an interdisciplinary team to assess needs, review labs, and adjust treatment plans to improve quality of life for patients and families.

Role requires an NP license, prescribing authority, Medicare billing privileges, and CPR certification. Experience in hospice or chronic care enhances candidacy.

Qualifications

• Current, unencumbered NP license in the state of employment.
• Minimum 1 year NP experience or hospice RN experience in the last 3 years.
• Authorized to practice and prescribe as NP in the service state.
• Must have Medicare billing privileges.
• Must meet state NP collaborative requirements and provide physician agreements.
• Must maintain required state insurance for NP practice.
• CPR certification.
• Knowledge of Medicare reimbursement and coding across care settings.

Responsibilities

• Communicate significant findings to the clinical manager and interdisciplinary team.
• Conduct face-to-face encounters and assess disease changes, function, adherence, and QoL.
• Order and review labs, X-rays, EKGs, and other tests; document treatment plans.
• Make referrals to other disciplines as needed.
• Participate actively in interdisciplinary team conferences.
