Job summary

Amedisys is seeking a nurse practitioner to join our home health team in a PRN capacity. You will perform comprehensive assessments, coordinate with physicians, and manage care plans in patients' homes.

Responsibilities include interpreting tests, refining treatment plans, and collaborating with an interdisciplinary team to improve patient outcomes. This role emphasizes flexibility and patient-centered care in a home setting.

Qualifications

• Current and unencumbered NP license in the assigned state.
• Minimum of one year as NP or hospice RN in last three years.
• Must have authority to practice and prescribe as NP in the assigned state.
• Medicare billing privileges as NP.
• Meet state NP collaborative requirements and provide agreements.

Responsibilities

• Communicate significant findings to the clinical team.
• Complete face-to-face encounters and assess disease changes, function, pain and QoL.
• Order and review labs, imaging, and diagnostics as indicated and document plans.
• Make referrals to other disciplines as needed.
• Participate in interdisciplinary team conferences.
