Job summary

Optimal Care is seeking an experienced Hospice After Hours Registered Nurse (RN) for West Michigan. You will deliver skilled nursing care and emotional support to patients and families during the overnight hours, a 7-on/7-off night rotation, with shifts from home and no on-call requirements.

You’ll travel within the service area to provide care in homes, assisted living facilities, or nursing homes, ensuring comfort and dignity at end of life while coordinating with physicians and hospice teams.

Qualifications

• Graduate of an accredited nursing program.
• Current State License as a Registered Nurse (MI).
• Minimum 1 year nursing experience in a clinical setting.
• Reliable transportation, valid driver’s license, insurance.
• Ability to work evenings, nights, weekends and holidays.

Responsibilities

• Assess and manage urgent symptoms during night hours.
• Provide skilled nursing interventions and support families.
• Coordinate with physicians and hospice team to ensure continuity of care.
• Work independently while maintaining safety and quality of care.
• Deliver compassionate care in patients' homes during overnight shifts.
