Job summary

Affinity Health Management is seeking a Medical Social Worker (MSW) to implement standards of care for hospice patients and their families. You will join the Interdisciplinary Group and provide psychosocial support based on assessments, goals, and care plans.

You will assess psychosocial needs at the patient’s residence, educate teams on culture and advanced directives, and participate in developing individualized plans of care, while maintaining clear clinical notes and coordinating with

Qualifications

• Master’s degree in Social Work (MSW) from a CSWE-accredited program.
• Minimum of one year experience in health care; hospice experience preferred.
• Experience in hospice care preferred.
• Good verbal and written communication, and strong organization skills.
• Offer contingent upon satisfactory references and criminal background checks.
• Prolonged walking/standing; adequate vision and hearing to perform social work duties.
• Must be a licensed driver with a valid insured vehicle.
• CPR certification required.

Responsibilities

• Assess psychosocial status of patients and families and report to the interdisciplinary group.
• Provide crisis intervention when necessary.
• Educate interdisciplinary team on cultural needs, respect for traditions and taboos.
• Assist in preparing advanced directives and care planning.
• Attend interdisciplinary meetings and support development of individualized care plans.
• Support patients and families in planning funeral arrangements and health care decisions.
• Maintain accurate clinical notes and coordinate with physicians and agencies.
• Participate in on-call duties and team improvement activities.
