Formación

• MSW from CSWE-accredited program required.
• Strong verbal and written communication skills essential.
• Good organizational abilities and case coordination experience.

Responsabilidades

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Provide crisis intervention and coordinate psychosocial support within the interdisciplinary group.
• Educate patients and families on advanced directives and cultural considerations.
• Participate in development of the individualized plan of care and attend meetings.
• Assist in planning for funeral, financial, legal, and health care decisions as needed.
• Maintain accurate clinical notes and coordinate with physicians and community resources.
