Job summary

You will assess patient and family needs, provide crisis intervention, and collaborate with the interdisciplinary team to develop and monitor individualized care plans.

Qualifications

• MSW from a CSWE-accredited program.
• Minimum 1 year of health-care experience; hospice experience preferred.
• Licensed driver; CPR certification required.
• Strong verbal and written communication skills.
• Ability to collaborate with interdisciplinary teams and manage psychosocial needs.

Responsibilities

• Assess psychosocial status of patients and families, communicating findings to the care team.
• Conduct social evaluations, including family dynamics and caregiver abilities; provide crisis intervention as needed.
• Educate on culture, advanced directives, and the plan of care; participate in interdisciplinary meetings.
• Develop individualized care plans with patient and family; monitor psychosocial needs and plan adjustments.
• Maintain accurate clinical notes; coordinate with physicians and community agencies; participate in on-call duties.
