Job summary

Apploi seeks a Medical Social Worker (MSW) to implement standards of care for hospice patients and families. As a core member of the Interdisciplinary Group, you will provide psychosocial support based on assessments and planned interventions, ensuring care aligned with the plan of care.

You will assess psychosocial status, educate teams on cultural needs, assist with advanced directives, and participate in developing individualized care plans.

Qualifications

• MSW from a CSWE-accredited program.
• Minimum one year of health care experience; hospice preferred.
• Experience in hospice care preferred.
• Excellent verbal and written communication, with strong organizational skills.
• Background checks and driving requirements may apply.
• Must be licensed to drive with insured vehicle.
• CPR certification required.

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the team.
• Provide crisis intervention and coping support; plan interventions.
• Educate interdisciplinary group on cultural needs and family dynamics.
• Assist in advance directive preparation and care planning.
• Attend interdisciplinary meetings and help ensure continuity of care.
• Support funeral, financial, and health care decision planning.
