Job summary

Affinity Hospice Management LLC is seeking a Medical Social Worker (MSW) to implement psychosocial care standards for hospice patients and families. You will be part of the Interdisciplinary Group, delivering support based on ongoing assessments and care plans, and coordinating with physicians and community resources.

The role emphasizes education of the care team on cultural needs, advance directives, and funeral/financial planning, ensuring compassionate, holistic end-of-life care.

Qualifications

• MSW from a CSWE-accredited program.
• Minimum of one year of health care experience; hospice experience preferred.
• Experience in hospice care preferred.
• Excellent verbal and written communication, and organizational skills.
• Contingent upon satisfactory references and criminal background checks.
• Must have a valid driver’s license and insured vehicle.
• CPR certification required (hands-on verification).

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Conduct social evaluations, plan interventions, and provide crisis intervention as needed.
• Educate the interdisciplinary group on cultural needs and family dynamics.
• Assist in preparing advanced directives and develop the individualized plan of care.
• Attend interdisciplinary meetings and support the team in psychosocial aspects of care.
• Support the physician and team in understanding social factors related to health and death.
• Assist families with planning for funeral, financial, legal, and health care decisions.
• Complete and maintain clinical notes; coordinate with physicians and community agencies.
