Qualifications

• MSW from CSWE-accredited program.
• Minimum 1 year healthcare/hospice experience.
• Experience in hospice care preferred.
• Strong written and verbal communication.
• Valid driver’s license and insured vehicle.
• CPR certification required.

Responsibilities

• Assess psychosocial status of patients and families; communicate findings to the care team.
• Conduct social evaluations and plan interventions; provide crisis intervention as needed.
• Educate on culture and advanced directives; develop the individualized plan of care.
• Collaborate with interdisciplinary group; support emotional needs of patient and family.
• Assist with funeral planning, financial and legal considerations.
• Complete and maintain accurate clinical notes; coordinate with physicians and community agencies.
