Job summary

Affinity Hospice Management LLC is seeking a Medical Social Worker (MSW) to implement standards of care for hospice patients and families. You will join the Interdisciplinary Group to provide psychosocial support, based on ongoing assessments and goals, in accordance with the care plan.

You will assess, educate, and assist with advanced directives, family dynamics, and crisis intervention. The role requires cultural awareness, collaboration with the team, and ongoing professional development.

Qualifications

• Graduate degree: MSW from CSWE-accredited program.
• Strong verbal and written communication abilities.
• Good organizational and time-management skills.
• CPR certification required.
• Licensed driver with vehicle.
• Background checks required after offer.

Responsibilities

• Assess psychosocial status of patients and families in hospice; communicate findings to the care team.
• Conduct social evaluations, address family dynamics, caregiver abilities, and risk factors; provide crisis intervention as needed.
• Educate interdisciplinary team on cultural considerations and advanced directives.
• Contribute to development of individualized care plans with patient and family input.
• Assist physician and team in addressing social and emotional factors affecting health and death/dying.
• Plan for funeral arrangements, finances, legal duties, and healthcare decisions.
• Document clinical notes accurately; maintain communication with physicians and community partners.
• Coordinate information and referrals; liaise with community agencies and support networks.
• Participate in on-call duties and ongoing education; uphold quality improvement efforts.
• Maintain safe environment and continuity of care for patients and families.
• Comply with reporting requirements for mandated reporters; adhere to policies.
