Qualifications

Responsibilities

• Assess psychosocial status of patients and families/caregivers and communicate findings to the care team.
• Conduct social evaluations including family dynamics and caregiver abilities; plan interventions.
• Educate on advanced directives and cultural considerations within care plans.
• Participate in development of individualized care plans with patient and family.
