Affinity Hospice Management LLC in Florida is seeking an in-person Medical Social Worker (MSW) to implement standards of care for hospice patients and families. You will be part of the Interdisciplinary Group, providing psychosocial support based on assessments and an individualized plan of care.

Responsibilities include assessing psychosocial status, conducting social evaluations, educating the team on cultural needs, guiding advanced directives, attending meetings, and supporting families

Qualifications

• MSW from a CSWE-accredited program.
• Minimum of 1 year health care experience; hospice experience preferred.
• Licensed driver with insured vehicle; CPR certification.

Responsibilities

• Assess psychosocial status of patients and families; communicate findings to the care team.
• Conduct social evaluations; provide crisis intervention as needed.
• Educate the care team on cultural needs and advanced directives.
