Job summary

Affinity Hospice Management LLC is seeking a Medical Social Worker (MSW) to implement standards of care and provide psychosocial support for hospice patients and families, coordinating with the interdisciplinary team in Ohio.

The role involves assessments, crisis intervention, education of care teams on cultural considerations, and helping with advanced directives and care planning. A CSWE-accredited MSW and CPR certification are required, with hospice experience preferred.

Qualifications

• Master’s in Social Work (MSW) from a CSWE-accredited program.
• Minimum of one year experience in health care; hospice experience preferred.
• Hospice experience preferred.
• Strong verbal and written communication and organizational skills.
• Contingent upon satisfactory references and background checks after offer.
• Must have a licensed driver with a valid insured vehicle.
• CPR certification required (hands-on verification preferred).

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Conduct social evaluations and plan interventions based on assessments.
• Educate interdisciplinary team on cultural needs and family dynamics.
• Assist in advanced directives preparation and plan of care.
• Participate in interdisciplinary meetings and support emotional well-being of patients and families.
• Assist with funeral planning, financial, legal, and health care decision-making.
