Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in patients' homes and communities in Poulsbo, Washington. The RN will use the nursing process to assess needs and coordinate care with the interdisciplinary team, patients, and families.

The ideal candidate has current RN licensure, strong communication, and the ability to develop and adjust care plans, educate families, and provide compassionate, competent nursing in home-based hospice

Qualifications

• Graduate of an accredited school of nursing.
• RN with license in good standing to practice.
• BSN with two years' experience or RN with one year in last three years, preferably in public health/home health/hospice.
• Management experience not required; may supervise aides.
• Offer contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication, problem solving, and nursing skills per competency checklist.
• Prolonged walking/standing; ability to lift and transfer patients; good vision and hearing.
• Must be a licensed driver with insured vehicle in good order.
• CPR certification required (hands-on verification).

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Assesses and evaluates patient status by writing/initiating and revising the plan of care as needed.
• Initiates the plan of care and revises it as patient status changes.
• Uses health data to determine nursing diagnoses and develop care plans with patient/family involvement.
• Implements preventive and rehabilitative nursing procedures and administers physician-prescribed medications.
• Counsels patient and family in meeting nursing and related needs.
• Provides health care instructions and assists with activities of daily living to promote independence.
• Acts as Case Manager when assigned and coordinates patient care for caseload.
• Participates in on-call duties and ensures equipment/services are available.
