Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to lead planning, organization, and direction of hospice care in patients' homes and communities in Tacoma, WA. The RN will perform comprehensive assessments, develop and revise care plans, and coordinate with the interdisciplinary team to meet patient and family needs.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse license in good standing.
• Experience requirement: 1 year of professional nursing in the last 3 years; or a BSN with 2+ years in public health, home health, or hospice nursing.

Responsibilities

• Performs comprehensive initial and ongoing patient/family assessments to determine hospice needs.
• Provides professional nursing care utilizing the nursing process.
• Develops, initiates, and revises the plan of care with patient/family input.
• Administers medications and treatments as prescribed.
• Acts as Case Manager when assigned and coordinates patient care for the caseload.
• Educates patient and family on self-care, medications, diet and care techniques.
• Ensures a safe environment and continuity of care across the interdisciplinary team.
