Job summary

Affinity Health Management is seeking a registered nurse to plan, organize, and direct hospice care for patients in home settings and communities in Washington. The RN will assess patients and families, develop care plans, and coordinate with the interdisciplinary team to ensure compassionate, comprehensive care.

The ideal candidate will be a graduate of an accredited nursing program, hold an RN license in good standing, and have CPR certification.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in good standing to practice.
• Minimum of 1 year of professional nursing experience within the last 3 years or 2 years with a Baccalaureate in nursing with at least 1 year in public health, home care, or hospice preferred.
• Must have a valid driver’s license and a vehicle in good working order.
• CPR certification (may require hands-on verification).

Responsibilities

• Completes initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Develops and revises the plan of care with the interdisciplinary team.
• Administers medications and treatments as prescribed and counsels patient and family.
• Maintains accurate clinical notes and communicates changes in patient condition to physicians.
• Acts as Case Manager for assigned caseload and coordinates care with the family and hospice team.
