Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in community/home settings. The RN conducts comprehensive patient/family assessments and develops individualized care plans using the nursing process.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises the plan of care with patient/family involvement as needs change.
• Initiates and updates care plans; administers physician-ordered medications and treatments.
• Teaches patient and family self-care techniques and coordinates with the interdisciplinary team.
