Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in patient homes and communities. The RN builds from community resources to meet patient and family needs with emphasis on education and holistic care.

The role includes full nursing responsibilities, case management for assigned caseload, and collaboration with an interdisciplinary team to ensure quality, compassionate hospice services.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• Minimum of 1 year of nursing experience within last 3 years; or BSN with NLN accreditation and RN license.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Completes comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Initiates and revises the plan of care as patient needs change.
• Administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing and related needs.
• Maintains accurate clinical notes and documents changes.
• Participates in on-call duties and coordinates equipment/supplies.
• Acts as Case Manager for assigned caseload when required.
