Job summary

Affinity Health Management is seeking a registered nurse to plan, organize and direct hospice care within home and community settings, with emphasis on compassionate nursing and family education.

The role involves comprehensive patient assessments, developing and revising care plans, administering prescribed treatments, documenting care, and coordinating with physicians and interdisciplinary teams to meet emotional and physical needs.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice in the state.
• 1 year of experience as a professional nurse within the last 3 years; or BSN with 2 years’ experience (public health/home health/hospice preferred).
• Excellent observation, verbal and written communication skills, problem solving and basic math skills; nursing skills per competency checklist.
• Must be a licensed driver with insured vehicle in good working order.

Responsibilities

• Conduct comprehensive patient and family assessments to determine hospice needs.
• Develop, implement and revise the plan of care with patient and family involvement.
• Administer medications and treatments as prescribed and monitor patient status.
• Provide health education and counseling to patients and families.
• Document clinical notes accurately and communicate with physicians and care teams.
• Participate in on-call duties and interdisciplinary team meetings.
