Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver person-centered care in patients' homes. You will assess needs, implement the nursing process, and collaborate with the Interdisciplinary Group to craft and direct individualized care plans.

Responsibilities include comprehensive assessments, Plan of Care development, on-going evaluation, medication administration as prescribed, and on-call participation, with emphasis on comfort and dignity at end-of-life.

Qualifications

• Associate or Bachelor's degree from an accredited School of Nursing.
• 1 year experience practicing as a registered nurse preferred.
• Current, unencumbered RN license.
• National Provider BLS certification within 30 days of hire (American Heart Association).

Responsibilities

• Assess patient and family needs to determine hospice requirements.
• Provide comprehensive physical assessments and nursing care using the nursing process.
• Write and initiate Plan of Care with the IDG and update as patient needs evolve.
• Regularly reevaluate patient/family/caregiver needs and revise the Plan of Care accordingly.
• Administer medications and treatments as prescribed by the IDG Plan of Care.
• Participate in on-call duties and maintain productivity per guidelines.
• Maintain accurate clinical notes and documentation of patient condition and care.
• Supervise ancillary personnel and participate in team orientation and development.
