Job summary

Providence at Home with Compassus is seeking a compassionate Hospice Registered Nurse (RN) to deliver person-centered home-based care. You will assess patient and family needs, provide nursing care per the nursing process, and collaborate with the Interdisciplinary Group to develop individualized care plans.

You will participate in on-call duties and maintain productivity standards to support end-of-life care with dignity.

Qualifications

• Requires Associate or Bachelor's degree in Nursing.
• Preferred: 1 year of hospice RN experience or hospice CNA/LPN background.

Responsibilities

• Assess patient and family needs and develop care plans with the IDG.
• Provide professional nursing care per the nursing process in the home setting.
• Write and initiate Plan of Care with IDG and revise as patient status changes.
• Participate in on-call duties and maintain required productivity.
