Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver person-centered care in home settings. You will assess patient and family needs, provide professional nursing care, and collaborate with the IDG to develop and direct individualized plans of care.

The role emphasizes compassion and end-of-life care, with responsibility to write and adjust plans of care, perform physical assessments, and participate in on-call duties as required.

Qualifications

• Associate or Bachelor’s degree in Nursing from an accredited program.
• Current RN license in good standing.
• Hospice or home health experience preferred (1 year).

Responsibilities

• Completes assessment of patient and family to determine hospice needs.
• Writes and initiates Plan of Care in collaboration with the IDG.
• Regularly reevaluates patient/family needs and revises the Plan of Care as necessary.
• Initiates the Plan of Care and makes revisions as patient status changes.
• Develops a care plan with patient/family/IDG and IDG to meet nursing goals.
• May administer medications and treatments as prescribed by the IDG.
