Job summary

Providence at Home with Compassus seeks a dedicated Hospice Registered Nurse (RN) to deliver person-centered home-based care. You will assess needs, provide nursing care, and work with the Interdisciplinary Group to plan and direct individualized care plans for patients and families facing end-of-life care.

Required an RN license and an Associate or Bachelor's degree in Nursing; BLS within 30 days. Hospice experience preferred; CNA/LPN in hospice will be considered.

Qualifications

• Associate or Bachelor's degree in Nursing from an accredited program.
• Current RN license in the state of employment.
• BLS certification within 30 days of hire.
• Hospice nursing experience preferred; CNA/LPN hospice experience considered.

Responsibilities

• Assess patient and family needs and develop care plans with the IDG.
• Provide professional nursing care grounded in the nursing process.
• Administer medications and treatments as prescribed.
• Collaborate with IDG to direct individualized plans of care.
• Participate in on-call duties and maintain productivity standards.
• Document clinical notes accurately and timely.
