Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver compassionate, home-based care. In partnership with the Interdisciplinary Group (IDG), you will assess patient and family needs and implement a Plan of Care grounded in the nursing process.

You will perform complete physical assessments, write and revise care plans, administer medications as prescribed, participate in on-call duties, and maintain accurate clinical documentation.

Qualifications

• Current, unencumbered RN license in the state of employment.
• Nursing degree from an accredited program is required.
• Hospice experience or CNA/LPN background considered favorable.

Responsibilities

• Completes assessment of patient and family to determine hospice needs.
• Writes and revises Plan of Care in collaboration with the IDG.
• Administers medications and treatments as prescribed by the IDG.
• Participates in on-call duties and maintains accurate clinical notes.
