Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver compassionate, person-centered care in patients' homes. You will assess needs, perform professional nursing care, and collaborate with the IDG to direct individualized plans of care for diverse age groups.

The role emphasizes teamwork, educational growth, and adherence to nursing processes while supporting families during end-of-life care. A valid RN license and BLS are required within 30 days of hire.

Qualifications

• Must hold an active RN license
• BLS certification required within 30 days of hire
• Experience in hospice or home health care preferred

Responsibilities

• Completes assessment of patient and family to determine Hospice needs.
• Provides a complete physical assessment and history and professional nursing care using the nursing process.
• Writing and initiating Plan of Care in collaboration with the IDG.
• Regularly reevaluating patient/family/caregiver needs and revising the Plan of Care as necessary.
• Initiates the Plan of Care and makes revisions as patient status changes.
• Develops a care plan with patient/family/IDG based on nursing diagnosis and palliative actions.
• May administer medications and treatments as prescribed by the IDG Plan of Care.
• Participates in on-call duties as defined by on-call procedures.
• Maintains productivity standards as set by organization guidelines.
