Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver person-centered care in the home setting. You will assess and plan care with the IDG, administer treatments, and support patients and families with compassion and clinical expertise.

The role emphasizes collaboration with the IDG, ongoing plan adjustments, and on-call duties while maintaining productivity and quality standards. This is a full-time, day-shift position in a home-based hospice environment.

Qualifications

• Associate or Bachelor's degree in Nursing is required.
• Current, unencumbered RN license is required.
• Within 30 days of hire, BLS certification is required.

Responsibilities

• Complete assessment of patient and family to determine hospice needs.
• Write and initiate Plan of Care with IDG.
• Reevaluate patient/family needs and revise Plan of Care as needed.
• Administer medications and treatments per Plan of Care.
• Participate in on-call duties.
• Maintain productivity as per guidelines.
