Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver patient-centered care in patients' homes. You will assess needs, perform comprehensive nursing care, and collaborate with the IDG to develop care plans aimed at comfort and dignity in end-of-life care.

The role requires an RN license with current BLS certification; 1 year hospice experience preferred. Per diem, day shifts, with on-call as needed; benefits package available.

Qualifications

• Associate or Bachelor's degree from an accredited School of Nursing.
• Preferred - 1 year experience practicing as a registered nurse; Hospice CNA/LPN experience considered.

Responsibilities

• Assess patient and family to determine hospice needs.
• Provide complete physical assessments and nursing care per the nursing process.
• Develop and initiate Plan of Care with the Interdisciplinary Group (IDG).
• Regularly reevaluate patient/family needs and revise the Plan of Care.
• Administer medications and treatments as prescribed by the IDG Plan of Care.
