Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver compassionate, patient-centered care in home settings. You will assess needs, provide nursing care, and collaborate with the IDG to direct individualized plans of care for patients at end of life.

This role emphasizes community health, education, and supporting families as patients live with dignity and comfort. Weekend shifts are part of the schedule with a focus on quality end-of-life care.

Qualifications

• Current, valid RN license in the required state.
• Education in nursing (AS/BSN) from an accredited program.
• Preferred hospice experience or CNA/LPN background considered.

Responsibilities

• Assess patient and family needs and develop an individualized plan of care.
• Provide professional nursing care in the home using the nursing process.
• Collaborate with the IDG to update Plans of Care and document care.
• Participate in on-call duties and maintain productivity standards.
